Medical Advocacy Ministry (MAM)

What is the Medical Advocacy Ministry of Grace?

We are here to serve you by assisting with late in life healthcare issues including health education, aging in place, end of life issues, senior support and navigating the health care system.

Some Specific Areas of Assistance Available

•  Healthcare Education

•  Aging in Place — Making your home safe for living in your late years

•  End of Life Issues — Advance Directives (Living Wills), Healthcare Power of Attorney, Palliative and Hospice Care

•  Senior Support —  Home healthcare, Caregiver Resources

•  Healthcare Provider and Facility Navigation —  Resources for locating a physician, healthcare facility, insurance

In Case of Emergency (I.C.E.) Document

An I.C.E. document is a summary of your medical history. This is important for a number of reasons:

•  When you change to a new primary care physician

•  When you visit a specialist (including your dentist)

•  When you call 911 to show the EMT responders

•  When you are admitted to an Emergency Room

You can carry a paper copy of this in your wallet or purse, or you can enter the information on the health app in your mobile phone. You should also give your significant other or Emergency Contact a copy in case yours is not readily available. Below is a button link to a downloadable blank form for easy completion.
